Job summary

United and Empowered in Lynchburg, VA seeks a Direct Support Professional to assist adults in Day Support or Community Engagement, providing hands-on care and support tailored to each individual’s goals.

You will collaborate with a team, follow person-centered plans, document progress, escort participants to activities, and help maintain a safe, clean environment while attending trainings and staff meetings.

Qualifications

• High school diploma or GED or 2 years of documented experience with individuals with intellectual disabilities.
• Requires successful completion of criminal history and central registry background investigations
• Valid Driver's License

Responsibilities

• Direct responsibility for guiding adults in Day Support or Community Engagement.
• Responsible for the physical care of individuals.
• Help individuals develop acceptable habits and attitudes.
• Assist with management of individual behavior per service or behavioral plan.
• Ensure that daily needs of individuals served are met fully.
• Escort individuals to and from community activities.
• Conduct head counts at start, during, and end of day to verify presence and well-being.
• Follow procedures for reporting behavioral changes or concerns, cleanliness, injuries/conditions, and demeanor.
• Complete data collection per person-centered plan.
• Perform housekeeping duties to maintain a clean, safe environment.
• Attend staff meetings and in-service training.
• Plan and implement special activities and complete special projects.

Education

Direct Support Professional

• Performs activities and duties to help individuals develop acceptable habits and attitudes.
• Assists with the management of individual behavior, including helping the individuals meet the outcomes of any service or behavioral plan.
• Ensure that the daily needs of the individuals served are met fully.
• Escorts individuals to and from community activities.
• Conduct a head count at the beginning, ending, and periodically throughout the day to verify the presence and well-being of individuals.
• Follow established procedures for reporting behavioral changes or other concerns, cleanliness, physical injuries/conditions, and demeanor.
• Complete data collection in accordance with the person-centered plan,
• Perform required housekeeping duties to assure a clean, safe, and healthful environment.
• Attend staff meetings and in-service training as assigned.
• Plan and implement special activities, and
• Complete special projects and other activities assigned.

Requirements:

• Have a high school diploma, or GED, or 2 years of documented experience working with a family member, a friend, or other persons with intellectual disabilities in a nursing home, facility, group home, or in-home setting.
