Job summary

Ozarks Community Health Center (OCHC) seeks a Driver to transport patients to appointments within the Care Team. This role supports the mission of OCHC by ensuring timely rides and clear communication about transportation, insurance, and fees.

The driver will coordinate routes, document interactions, and refer patients to care resources as needed, while participating in meetings and trainings to support patient access and continuity of care.

Qualifications

• High school diploma or equivalent required.
• Bus driver physical at time of hire, renewed every 2 years.
• Class E Driver's License or CDL with passenger endorsement.
• BLS (Basic Life Support) within 6 months of employment.
• Driving experience preferred.

Responsibilities

• Schedules transports and ensures rides are correct in EMR and MTM.
• Keeps appointments and picks up patients in a timely manner.
• Communicates with patients regarding their transportation.
• Confirms patients' insurance and communicates all trip fees in advance of the ride.
• Refers patients to the Sliding Fee Scale when appropriate.
• Coordinates with staff and patients to reschedule or cancel clinical appointments as needed.
• Communicates with MTM, including changing and cancelling trips and resolving issues.
• Coordinates efficient routes to maximize patients assisted daily; submits claims to MTM timely and accurately.
• Refers patients to CHWs, care management, and other clinic resources as needed.
• Participates in meetings, trainings, and activities.
• Keeps providers and care team informed of barriers to care and any other pertinent patient information.
• Appropriately documents all patient interactions, including phone calls and rides.
• Other duties as assigned

Education & Training

• Bus driver physical at the time of hire, renewed every 2 years (Sooner frequency might be necessary per provider)

Licensure & Certifications

• Class E Driver's License or CDL with passenger endorsement
• BLS (Basic Life Support) within 6 months of employment

Experience
