Job summary

EveryAge is hiring a Part-time Driver for its PACE transportation program in New Bern, NC. Under supervision of the Transportation Coordinator, you will safely transport PACE participants, assist with boarding, and ensure daily vehicle safety and cleanliness.

You will follow schedules, communicate changes to dispatch, and maintain required records. A high school diploma is required; CPR/First Aid is preferred, and a valid driver's license with insurance is necessary.

Qualifications

• High school diploma or equivalent.
• CPR/First Aid certification preferred.
• Must have a valid driver's license and proof of insurance.
• Minimum age 23 for insurance purposes.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Minimum one year experience with frail or elderly population.

Responsibilities

• Transport EveryAge PACE participants safely and on time.
• Maintain cleanliness and safety of assigned vehicle.
• Complete daily logs and inspections.
• Coordinate changes with dispatch.
• Assist participants on and off vehicle.
• Communicate participant condition changes to Transportation Coordinator.
• Attend required trainings and maintain certifications.
• Perform other duties as required.
• Follow plan of care as directed by Interdisciplinary Team.

PACE (Program of All Inclusive Care for the Elderly) programs operate in 33 states with the mission to provide comprehensive, personalized healthcare services including primary care to elderly people 55+ who are at risk for needing placement in a skilled nursing facility. A primary goal of PACE is to keep these vulnerable individuals in the community setting so long as they can safely be cared for in the community.

Driver, Part-time

Schedule Requirement: M-F (1pm-6pm)

Under the supervision of the Transportation Coordinator, the Driver is responsible for the safe and timely transportation of EveryAge PACE participants. Including transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for overall cleanliness and safety of the vehicle assigned on a daily basis. Responsible for communicating any safety issue to the Transportation Coordinator.

Essential Duties and Responsibilities:

• Responsible for EveryAge PACE passengers during transport, providing care in a manner which ensures participant satisfaction.
• Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to dispatch.
• Maintains and completes daily required records i.e., Pre- and Post-Trip Inspections, gasoline purchases.
• Transports participants to and from medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care.
• Provides physical assistance to each participant when needed as he/she boards and debarks from the program’s vehicles.
• Communicates with Transportation Coordinator daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests.
• Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses
• Performs other duties as required or requested.
• High school diploma or equivalent required.
• Cardiopulmonary Resuscitation (CPR)/First Aid certification preferred.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage which requires a minimum age of 23 years old.

Minimal Experience:

A minimum of one year of experience in working with the frail or elderly population required.
