Job description

SUMMARY: Under the supervision of program management, the Integration Assistant will provide community-based instruction and personal care to persons with disabilities.

QUALIFICATIONS:

1. Education/Experience: High school diploma or GED, with a preference for individuals who have experience working in day or residential programs serving persons with disabilities.
2. Language Ability: Ability to read, write, and understand English. Strong oral and written communication skills are required. Ability to understand and follow verbal and written instructions from supervisors.
3. Math Ability: Knowledge of and ability to work with basic mathematical concepts.
4. Computer Skills: Working knowledge of personal computers is highly desirable.
5. Driving Skills: Must possess a valid CDL and have a clean driving record.

DUTIES:

1. Provide community-based group instruction (1:3) based on individual ISP's.
2. Record daily data on consumers' progress.
3. Maintain communication with Program Manager, Instructor, and/or Program Assistant.
4. Ensure consumers' health and safety in both facility and community settings.
5. Assist with behavioral modifications as per ISP.
6. Contribute to the development of individualized program objectives.
7. Provide personal care, including toileting, as needed.
8. Promote a teamwork-oriented environment.
9. Attend training and staff meetings.
10. Maintain equipment as required.
11. Provide transportation to consumers daily.
12. Perform other duties as assigned.
