Qualifications

Job description

• Conduct outreach, check-ins, and comprehensive assessments to identify and meet patient needs via phone calls, SMS, video calls, or in-person encounters
• Assist patients with appointment setting, office visits, social services applications, and other requests
• Coordinate with health plans, service providers, and multidisciplinary care teams to advocate for patient needs and drive services to completion
• Use motivational interviewing, trauma-informed care, and harm reduction to engage patients and support positive habit change
• Use required care management portals to document care coordination and services in a timely manner
• Meet patient caseload targets and prioritize services according to intensity, need, and required follow-up
• Conduct in-person outreach at WIC clinics, FQHC waiting rooms, and CBO program sites across assigned counties
• Conduct virtual outreach through social media and other online spaces
• Help organize or staff monthly pop-up events, baby supply giveaways, community baby showers, and clinic tabling days
• Identify opportunities for Malama to highlight services at community events hosted by other organizations

Requirements

• 1-2 years of experience as a Community Health Worker, Community Doula, Case/Care Manager, or Social Worker
• Must be a certified birth doula prior to employment, including training, attendance at 3 births, and completed CPR training
• Must enroll in Health First Colorado Medicaid
• Reliable and responsive to text and calls
• Adaptable and excited to learn new technology
• Comfortable using Google Workspace (Sheets, Docs, Drive), Notion, Slack, and Zoom
• Positive and empathetic when serving members/clients
• Open and honest when providing internal feedback for improvement
• Passionate about health, technology, Malama's mission, maternal health outcomes, and closing equity gaps
• Deep ties to and lived experience in the communities served
• Ability to travel regularly across assigned counties for partner site visits
• Comfortable engaging with CBOs, following up, and tracking outreach activities
• Prior experience working with Medicaid and/or community-based maternal health programs
• Fluent in English, written and verbal
• Bilingual in Spanish, written and verbal
• Located in the assigned Colorado service area
