Job summary

Malama Health is seeking a Community Doula to serve as a Maternity Care Navigator, conducting outreach and coordinating services with patients, providers, and community partners in Colorado.

You will use mobile and cloud tools to document care and track progress, travel across counties, and participate in events with WIC clinics and CBOs. English fluency is required, with Spanish bilingual skills strongly preferred.

Qualifications

  • 1–2 years of experience as a Community Health Worker, Community Doula, Case/Care Manager, or Social Worker.
  • Must be a certified birth doula prior to employment and enroll in Health First Colorado Medicaid.
  • Fluent in English (written and verbal) and bilingual in Spanish.

Responsibilities

  • Deliver Care Management to Meet Patient Needs via calls, SMS, video, or in-person visits.
  • Coordinate care with health plans, providers, and care teams to completion.
  • Use motivational interviewing and trauma-informed approaches to empower patients.
  • Document and track services using care management portals and meet caseload targets.
  • Conduct in-person and virtual Community Outreach to engage partners and events.

Job description

Malama leverages the best of technology and people to deliver exceptional care - we build novel biometric tracking and patient engagement tools that allow skilled Care Managers to provide high-touch care navigation and coordination for pregnant and postpartum mothers that improves health outcomes for mom and baby.

We’ve raised $9.2M in funding to date and are on target to hit profitability early this year. Our mission is to improve pregnancy-related outcomes beginning with conditions that disproportionately affect historically underserved groups.

We’re seeking a Community Doula to use our tools to serve our members as their Maternity Care Navigators and build awareness of Malama in their local region. This role is pivotal in bridging the gap between providers, community-based organizations and patients, ensuring seamless execution of our policies and care protocols. If you’re passionate about improving maternal health outcomes, comfortable doing outreach, love building relationships, closing equity gaps, and want to gain invaluable experience in care coordination, startup operations, and patient success, this position is for you.

Role Responsibilities
  • Deliver Care Management to Meet Patient Needs - Conduct outreach, check-ins, and comprehensive assessments to identify and meet patient needs via phone calls, SMS, video calls, or in-person encounters; Assist patients with appointment setting, office visits, social services application, among other requests.
  • Coordinate Care Services and Referrals to Completion - Advocate for patient needs by coordinating with health plans, service providers, multi-disciplinary care teams to drive services to completion.
  • Communicate to Empower Patient Learning - Utilize evidence-based practices like motivational interviewing, trauma-informed care, and harm reduction to engage with and communicate information to patients towards positive habit change.
  • Utilize Technology to Document, Track, and Improve Services - Utilize required care management portals for all care coordination and other services provided in a timely manner; meet patient caseload targets and prioritize services according to intensity, need, and required follow-up.
  • Community Outreach & Relationship Building - Conduct in-person outreach at WIC clinics, FQHC waiting rooms, and CBO program sites across assigned counties, virtual outreach and engagement via social media and other online spaces, help organize or staff monthly pop-up events, baby supply giveaways, community baby showers, and clinic tabling days and identify opportunities for Malama to highlight services at community events hosted by other organizations.
  • 1-2 years of experience as a Community Health Worker, Community Doula, Case/Care Manager, Social Worker
  • Must be a certified birth doula prior to employment (training, attendance at 3 births, complete CPR training, etc.) and enroll in Health First Colorado Medicaid
  • Reliable and responsive to text and calls! Adaptable and Excited to learn new technology - You are comfortable using Google Workspace (Sheets, Docs, Drive), Notion, Slack, Zoom, and want to learn more

  • Prior experience working with Medicaid and/or community-based maternal health programs
