Job summary

Westside Campus of Care in Texas is seeking a Licensed Vocational Nurse (LVN) for a double-weekend shift (6A-10PM, Sat/Sun) to deliver compassionate patient care to long-term residents and rehabilitation patients. You will collaborate with the interdisciplinary team to implement individualized care plans that promote recovery and well-being.

The role requires a Texas LVN license and a high school diploma; prior SNF/rehab experience is a plus.

Qualifications

• Current LVN license in the State of Texas.
• High school diploma or equivalent required.
• Previous experience in a skilled nursing or rehabilitation setting preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Provide licensed nursing care to patients, including administering medications, wound care, and other treatments as ordered by physicians.
• Conduct patient assessments, monitor vital signs, and report changes to physicians and other healthcare professionals.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Coordinate patient care with other healthcare providers, including home health and hospice services.
• Participate in quality improvement initiatives and patient satisfaction surveys.

About the Role:
