Eckerd Connects is hiring a part-time Diversion Case Manager in CenLA JJ located in Alexandria, LA. The role focuses on assessing youth, creating individualized intervention plans, and coordinating with families and stakeholders to support positive outcomes.

The DCM provides supportive counseling, maintains case records, and ensures program goals are met while complying with agency policies. The position involves travel as needed within the program scope.

Qualifications

  • Bachelor's degree required or 5+ years of relevant experience may substitute for the degree.
  • Pass drug screen per workplace policy.
  • Must be able to drive for business purposes.

Responsibilities

  • Conduct a comprehensive assessment to develop Individualized Intervention Plans for youth.
  • Serve as liaison between provider, youth, parents, OJJ, and other professionals; schedule and facilitate plan meetings.
  • Develop mutually agreed-upon Intervention Plans with short- and long-term goals; provide supportive counseling.
  • Complete administrative work including records, notes, and reporting within required timeframes.
  • Coordinate with community-based organizations to promote program visibility and meet outcomes.

Hourly Rate: $17.00

Part-Time Position : 20 hours weekly

Duties & Responsibilities
  • The Diversion Case Manager (DCM) works with youth who have been referred to the program and determined to be eligible for Juvenile Justice Diversion services.
  • The DCM is responsible for conducting a comprehensive assessment in order to develop Individualized Intervention Plans for youth.
  • Serves as the liaison between the Provider, and the youth, his/her parent(s), OJJ, and other service professionals; schedules and facilitates Individual Intervention Plan meetings.
  • This position guides the development of a mutually agreed-upon Individual Intervention Plan which defines short and long-term goals and orients youth to the program. The DCM provides supportive counseling to strengthen the youth’s ability to make appropriate life decisions; maintains regular contact with youth; monitors youth’s needs and progress on an ongoing basis; and updates the youth’s Individual Intervention Plan and records.
  • When community service is needed, the incumbent ensures the activity is reflective of the youth’s skills, experience, aptitude and interests to address some specific violation or behavioral concern.
  • The DCM completes all required administrative work including general clerical functions, maintenance of accurate, timely youth records; documenting case notes within established time-frames, and reporting.
  • This position serves as liaison to other community-based organizations to promote visibility of the program to ensure effective utilization of services and to meet program outcome goals.
  • DCM may assist in participant curfew checks and will provide transportation as needed and will comply with Eckerd’s Fleet Safety Program
  • The DCM shall maintain an average caseload of up to 25 youth.
Qualifications
  • Bachelor’s degree, from an accredited College or University, in criminal justice, social work, education, or OJJ approved related field; and a minimum of one year of professional experience working with at‑risk or delinquent youth.
  • 5+ years of professional experience in social services or education professional position working with at‑risk or delinquent youth may be substituted for a bachelor’s degree.
  • Must be able to meet requirements for Eckerd Connects Auto Insurance and be able to drive for business purposes.
  • Required to pass a drug screen in compliance with our Substance Abuse and Drug-Free Workplace policy.

About Our Program

Eckerd Connects’ Central Louisiana Juvenile Justice (CenLA JJ) program provides diversion services for youth engaged with the Juvenile Court. In partnership with the State of Louisiana Office of Juvenile Justice, our CenLA JJ team has helped young people build skills, confidence, and positive pathways while preventing deeper involvement in the justice system. Youth receive individualized support including mentorship, standardized assessments, goal-setting, pro‑social activities, and follow‑up support after discharge. We focus on strengths, not mistakes, helping youth move from uncertainty to confidence and success.
