The Role

As a Discharge Transportation Coordinator, you manage and coordinate patient transportation services across UVA Health using a variety of resources, including ambulances, wheelchair vans, taxis, public transportation, and other community-based options.

Working from UVA Health Town Center One, you serve asa central pointof contact for transportation planning, balancing patient needs, insurance requirements, transportation availability, and operational priorities to ensure efficient patient movement.

Your work includes:

  • Coordinatingpatient discharge transportationusing ambulances, wheelchair vans, taxis, buses, and other transportation resources
  • Communicating withpatients, families, clinical teams, case managers, and transportation vendorsto arrangeappropriate transportsolutions
  • Navigating insurance coverage requirements and transportation eligibility guidelines to support efficient discharge planning
  • Prioritizing transportation requests while managing multiple cases simultaneously in a dynamic healthcare environment
  • Investigating transportation concerns, resolving service issues, and supporting positive patient experiences
  • Tracking transportation activity, processing invoices, andmaintainingaccuraterecordsin accordance withdepartmental procedures
  • Supporting process improvement initiatives and assisting with trainingand onboarding of new team members

No two days are exactly alike. One moment you may be arranging a time-sensitive ambulance transfer for a patient with specialized medical needs; the next,you'recoordinating a discharge ride home while working through insurance coverage questions or transportation availability challenges.

You'llcollaborate closely with nurses, care coordinators, social workers, transportation providers, and community partners to keep patients moving safely and efficiently. PHYSICAL DEMANDS

This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.

The pay range for this role is $20.00 - $24.73 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
