Qualifications

• Masters in social work (MSW) or RN licensure required.
• Current NJ SW license or NJ RN license required; ACM/CCMC preferred.
• Experience in case management and discharge planning is desirable.

Responsabilités

• Provide psychosocial assessments and crisis intervention.
• Coordinate discharge planning with patient, families and the care team.
• Coordinate referrals to home care agencies, SNF, rehabilitation centers and community programs.
• Coordinate care authorization process with insurers.
• Coordinate hospitalization details with Utilization Management to ensure appropriate admission status.
