Job summary

UPMC Shadyside seeks a Senior Discharge Plan Manager to lead care coordination and discharge planning. You will oversee safe transitions from inpatient care to home or other settings, coordinating with patients, families, and providers to craft personalized discharge plans that address medical, social, and logistical needs.

The role emphasizes patient advocacy throughout discharge, ensuring concerns are heard and addressed, with a focus on timely, safe transitions and collaborative team care.

Qualifications

• RN license required.
• BSN required (10 years experience may substitute).
• MSW or master’s degree required (10 years experience may substitute).
• CCM/ACM or other nursing/social work certification preferred.

Responsibilities

• Coordinate discharge planning from admission to post-discharge.
• Advocate for patient/family access to services and protection of health and rights.
• Identify patient needs to guide discharge planning for optimal outcomes.
• Complete detailed assessments to determine self-care capacity and post-hospital needs.
• Collaborate with a multidisciplinary team to develop an individualized transition plan.
