Job summary

UPMC Mercy Health Center in Pittsburgh is hiring a Casual Discharge Plan Manager to support our Clinical Care Coordination and Discharge Planning Department. This role works varied days between 8am-4:30pm with weekend responsibilities, and invites an RN or social worker with care management interest.

The Discharge Plan Manager coordinates post-discharge needs, collaborates with the care team, and ensures a safe, timely transition for patients.

Qualifications

• RN or social worker with care management interests.
• Experience in case management or care coordination preferred.
• Ability to assess discharge needs and coordinate post-hospital care.

Responsibilities

• Identify clinical, psychosocial, financial, and spiritual needs to guide the planning process with the patient to attain optimal outcomes.
• Complete detailed assessment on every patient to establish understanding of medical and social factors and barriers to discharge.
• Facilitate teams to develop and execute safe and efficient discharges and coordinate post-hospital care.
• Engage in clear communication with patient/family/caregivers and the interdisciplinary care team to develop discharge plans.
• Serve as a liaison between hospitals, post-hospital facilities, and treating physicians.
• Demonstrate shared accountability in discharge planning with patient and team to ensure optimal outcomes.
• Align practice with organizational mission and regulatory requirements.
• Advocate for services access and patient rights.
• Manage cost of care while prioritizing safety, quality and patient satisfaction.
