Job summary

Beloit Health System is seeking a Care Coordinator / Inpatient Social Worker to advocate for patients and families, and to facilitate safe transitions of care within our interdisciplinary team.

The role requires a Bachelor's in Social Work (MSW preferred) and Wisconsin social work certification (temporary allowed). You will collaborate across care teams and coordinate resources to support patient-centered discharge planning.

Qualifications

• Bachelor's in Social Work required; MSW preferred.
• Wisconsin social worker certification (temporary acceptable).
• Knowledge of discharge planning and community resources.
• Ability to collaborate with a multidisciplinary team.
• Excellent communication and crisis intervention skills.

Responsibilities

• Conduct psychosocial assessments to identify needs.
• Develop individualized discharge plans for safe transitions.
• Coordinate community resources and post-acute services.
• Provide counseling, emotional support, and crisis intervention.
• Collaborate with physicians, nurses, and case managers.
• Assist with complex cases including abuse, homelessness, and financial hardship.
• Document assessments and progress in the EMR.
• Stay informed about regulatory requirements and hospital policies.
