Job summary

UPMC Shadyside is seeking an Expert Discharge Plan Manager to support Clinical Care Coordination and Discharge Planning. The role focuses on safe, patient-centered transitions from hospital to home or other care settings.

The ideal candidate will coordinate with providers, patients, and families to create personalized discharge plans addressing medical, social, and logistical needs. RN or MSW backgrounds encouraged.

Qualifications

• RN or social worker background with interest in care coordination
• Nurse Track requires BSN and RN license
• Non-Nurse Track requires MSW or a master's in a health/human services field
• CC/Discharge planning experience preferred

Responsibilities

• Perform in accordance with system-wide competencies/behaviors.
• Advocate for patient/family/caregivers to access services and protect health and rights.
• Develop individualized discharge plans addressing medical, social, and logistical needs.
• Collaborate with care team to coordinate post-hospital transitions.
• Lead process improvement initiatives in discharge planning.
