Discharge Planning Coordinator (UPMC Presbyterian and UPMC Shadyside)

Join us as a Discharge Planning Coordinator at UPMC Presbyterian and UPMC Shadyside. We are hiring full-time coordinators for each campus to support the Clinical Care Coordination and Discharge Planning Department.

Role Overview

The coordinator will assist with providing regulatory forms, post-acute placement, and communication with post-acute entities regarding patient discharge. The ideal candidate will have knowledge of medical terminology, excellent verbal and written communication skills, and be independent, self-motivated, and flexible.

Shift Details

Daylight shift with flexible hours between 7:30 am and 5:30 pm.

Responsibilities
  • Assist the Discharge Planning team with questions, troubleshooting, and coordination of patient discharge materials.
  • Order supplies and equipment based on clinical team needs.
  • Ensure compliance with regulatory and HIPAA requirements.
  • Perform clerical and non-clinical tasks to facilitate safe discharge plans.
  • Provide necessary forms and resources to patients and caregivers.
  • Communicate effectively and report concerns regarding care plans and discharge barriers.
  • Serve as a liaison between patients and discharge planning staff.
  • Utilize electronic medical records and technology for documentation and communication.
Discharge Planning Coordination
  • Coordinate clinical care needs from inpatient to outpatient, or from post-discharge facilities to home or assisted living.
  • Handle insurance authorizations, care level transitions, and resource connections.
  • Assist with organizing hemodialysis, hospice, and other specialized care services.
Qualifications
  • High School Diploma/GED or equivalent.
  • Excellent communication, organization, and multi-tasking skills.
  • Willingness to learn and take initiative.
  • Good time management and basic computer skills.
  • Ability to work independently and as part of a team.
Licensure and Certifications
  • National Certification (preferred).
  • Act 34 clearance.
