Job summary

UPMC is seeking a Discharge Planning Coordinator to support the Inpatient Discharge Planning team with clerical tasks, audits, and patient interviews. This role does not provide direct patient care but may interact with patients and families.

Ideal candidates have prior authorization experience and hospital patient care exposure, with proficiency in Outlook, scheduling, office tasks, and MS Office. Strong communication and organization are essential.

Qualifications

• High School Diploma or GED required or equivalent.
• Excellent communication skills with patients and families.
• Strong organization and prioritization abilities.
• Multitasking proficiency in a clinical/administrative setting.
• Willingness to learn and take direction.
• Ability to work both independently and as part of a team.

Responsibilities

• Performs clerical tasks delegated by Discharge Planning staff, and supports the discharge process.
• Provides forms, resources, and discharge information to patients/caregivers as directed.
• Uses electronic medical records and other systems to review charts, collect data, and document progress.
• Serves as a point of contact between patients and discharge planning staff.
• Gathers materials needed for discharge and orders supplies as identified by the clinical team.
• Adheres to system-wide competencies and HIPAA regulations.
• Reports concerns about the care plan and discharge barriers clearly and promptly.
• Assists the discharge planning team with questions and troubleshooting.
• Maintains professional demeanor and effective time management.
