Job summary

Johnson Memorial Hospital, part of Trinity Health Of New England, is seeking an LMSW Social Worker / Discharge Planner to join our Case Management team. The Social Worker will assess and provide direct social work services to patients and families in the hospital setting from admission through discharge.

The role is part-time with Day shift and per diem availability. Requirements include an MSW from an accredited school of social work; CT LMSW licensure preferred; CCM and ACCC certifications

Qualifications

• MSW degree from an accredited School of Social Work.
• CT State Social Work Licensure LMSW preferred.
• Certification in case management (CCM) and ACCC preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations.
• Experience with utilization management, home care, and rehabilitation beneficial.

• Assesses patient\'s needs and develops discharge care plans.
• Communicate effectively with staff, colleagues, patients, and family members.
• Utilizes standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.

Minimum Requirements:

• MSW degree from an accredited School of Social Work required.
• CT State Social Work Licensure LMSW preferred
• Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
• Knowledge of Medicare, Medicaid, and managed care regulations and trends

• Experience with utilization management, home care, community resources, and rehabilitation beneficial.
