Job summary

Saint Clare's Health, part of Prime Healthcare, is seeking a Case Management/Discharge planning professional in New Jersey to coordinate discharge plans, assist with referrals, and manage Medicaid submissions under the supervision of a Case Manager.

Experience in discharge planning, utilization management, and basic computer skills is preferred; a current BCLS certificate is required. Full-time Days shift with competitive benefits.

Qualifications

• Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions.
• Experience in basic to intermediate computer skills/knowledge.
• Current BCLS (AHA) certificate required upon hire and maintain current.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker and assist with discharge planning.
• Assist with case management referrals and authorizations.
• Maintain Medicaid state submission forms for presentation to on-site reviewer and files them per facility procedure.
