Job summary

Saint Clare's Health in Dover, NJ is seeking a Case Management Assistant to join its multidisciplinary team. This role supports discharge planning, coordination of case management activities, and referrals under the supervision of the Case Manager/Social Worker.

Qualified candidates have knowledge of discharge planning, utilization management, and case management terminology, plus basic computer skills and a current BCLS certificate.

Qualifications

• Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments.
• Experience in basic to intermediate computer skills/knowledge.
• Current BCLS (AHA) certificate required upon hire and maintain current.
• High School diploma required. Associates degree or LVN license preferred. Experience in the hospital setting preferred.

Responsibilities

• Coordinate the activities of the Case Management Department under the direction of the Case Manager/Social Worker; assist with development and implementation of discharge plans.
• Assist with case management referrals and authorizations.
• Maintain Medicaid state submission forms for presentation to on-site reviewer and file them accordingly.
• Assist with discharge planning by interviewing patients/families and gathering data to develop a safe patient-focused discharge plan.
