Qualifications

Responsibilities

• Coordinate and conduct discharge planning for patients with serious medical conditions.
• Develop and maintain relationships with community agencies for patient support.
• Identify release-planning needs and provide Life Skills Programs for patients.
• Assist patients with residential placement and medical needs.
• Document services in patient records and attend relevant meetings for care coordination.
