Job summary

Job description

Windsor Laurelwood is seeking a Discharge Planner.

Responsibilities

In this role, you will coordinate discharge plans for psychiatric inpatient and partial hospitalization patients, connect them with aftercare resources, and collaborate with an interdisciplinary team to ensure a smooth transition from care.

  • Coordinate and lead discharge planning, including scheduling follow-up appointments and communicating with aftercare providers.
  • Collaborate with referral sources such as SASS, DCFS, and other agencies.
  • Build therapeutic relationships with patients and provide ongoing support during their stay.
  • Educate patients and families on mental health conditions and treatment options.
  • Participate in interdisciplinary team meetings and clinical staffing.
  • Advocate for patients and contribute to program development initiatives.
  • Perform other duties as assigned.
Qualifications
