Job summary

ICFAI is seeking a Discharge Planner to assist patients and families with social, economic, or environmental challenges related to illness. The role is part-time with PRN shift coverage seven days a week, including some holidays, and involves coordinating with the care team for complex discharge planning.

Responsibilities include liaising with community agencies, ensuring access to health information, and supporting continuity of care across the post-acute spectrum.

Requirements

• Baccalaureate Degree in Social Work, psychology, sociology, or other related field.
• At least one year of healthcare discharge planning or social work experience is required.
• Must have working knowledge of community social and health agencies.
• Must have knowledge of human behavior and personality dynamics.

Essential Physical Requirements

• Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time.
• Must be able to lift and carry up to 30 pounds.
