Location: Port St Lucie Hospital
Position Summary

Port St Lucie Hospital is seeking an experienced and compassionate Discharge Planner. The Discharge Planner plays a vital role in ensuring an effective and safe discharge plan for patients transitioning from inpatient psychiatric care to community living by providing the support, resources, and continuity of care they need for long‑term stability and recovery.

Key Responsibilities
  • Interview patients, families and significant others, and evaluates the precipitating admission factors, emotional and psychological problems, available personal/community support systems, relevant family dynamics and financial/legal issues relative to discharge planning.
  • Formulate, implement, evaluate, and modify a clinically sound plan of care for patient discharge.
  • Assess and follow up on issues concerning patient’s rights as they relate to the Baker Act, emergency admissions procedures, involuntary placement procedures, etc.
  • Assure that patient’s or Guardian’s signature is obtained as indicated.
  • Educate patient or Guardian (if applicable) about legal rights as per Baker Act and has patient/Guardian sign same.
  • Formulate therapeutic relationships and provides education to patients, families, and individuals relative to discharge planning and aftercare.
  • Coordinate all discharge planning with families, interdisciplinary treatment team, agencies and other community resources.
  • Participate as a vital member of the interdisciplinary treatment team, agencies, and other community resources.
  • Interface with the family and physician in resolving problems associated with the patient’s admission and ongoing hospitalization.
  • Participate in establishing a full-range resource file of information pertinent to the treatment plan.
  • Complete and utilizes all relevant documentation.
  • Actively contribute to the development of Discharge Planner’s role within the department and hospital.
  • Performs special projects when requested or required.
  • Shows ability to communicate in a clear and concise manner.
  • Demonstrates ability to work with others.
  • Adapt to changing situations or work assignments.
  • Demonstrates willingness to rotate to other areas of hospital within the limits of preparation and skill level.
  • Conform to uniform and dress code, personal hygiene and good grooming.
  • Wear and properly display name badge.
  • Positively respond to guidance or counseling and attempts to benefit by it.
  • Adept in identifying potential problems within the department and seeks management guidance.
  • Demonstrates excellent organizational skills and originator of new ideas and methods.
  • Use verbal and non-verbal communication with others, i.e., courtesy, tone of voice, facial expressions, gestures, etc.
  • Maintain confidentiality.
  • Participate in continuing educational programs as prescribed by hospital policy, as well as attendance to hospital and department staff meetings per department policy.
Education and Experience
  • Bachelors Degree in medical field.
  • One to two years of clinical experience in a psychiatric setting.
  • Case Management Certification preferred.
