Job summary

College Medical Center Phoenix is seeking a full-time Discharge Planner to join its Case Management team in Phoenix, AZ. The role coordinates care across the continuum, working with physicians, social workers, and patients to develop effective discharge plans and ensure timely transfers and follow-up.

Requirements include a B.S. in Sociology or Psychology, at least one year in a behavioral health setting, and BLS certification with CPI training within 30 days.

Qualifications

• Bachelor of Science in Sociology or Psychology.
• Minimum of one year of experience in a behavioral health environment.
• BLS certification for healthcare providers required upon hire.
• CPI certification to be obtained within 30 days of hire.

Responsibilities

• Provide clerical support to Case Managers and Social Workers.
• Collaborate with physicians, therapists, treatment team members, and patients to develop effective continuing care plans.
• Assist with telephone calls, faxing, copying, and coordination of discharge plans.
• Present aftercare options to patients and discuss with individuals or families as needed.
• Collect, release, and manage patient information for research and referrals.
• Maintain timely and accurate patient records.
• Initiate contact and coordinate with referral sources, mental health providers, and care facilities to ensure continuity of care.
• Arrange admissions and transfers to hospitals, treatment centers, and other placements.
• Schedule follow-up appointments for outpatient treatment.
• Attend and contribute to treatment team meetings professionally and punctually.
• Build and maintain relationships with community resources and referrals.
• Conduct research and maintain updated provider and resource lists and databases.

Education

Bachelor of Science in Sociology or Psychology

Placement in the pay range is based on multiple factors, including but not limited to relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CMC Phoenix roles, including but not limited to shift differential and other special pay practices, etc. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CMC Phoenix in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
