Job summary

CVBH seeks a discharge planning professional to develop documented continuing care plans and coordinate post-discharge support for patients and families. You will ensure smooth transitions and connect patients with appropriate community resources.

The role requires an Associates degree in nursing or human services, with Bachelor's degree in social work or human services preferred, and 1+ year of clinical or discharge planning experience.

Qualifications

• Associates degree in nursing or human services required; Bachelor's in social work or human services preferred.
• One or more years’ experience in a clinical setting preferred.
• One or more years’ experience in discharge planning preferred.

Responsibilities

• Develop continuing after care plan, based on patient’s needs, to promote long term health and relapse prevention.
• Set discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.
• Make arrangements for community resources to facilitate discharge; provide documentation to other providers to ensure continuity of care.
• Act as a liaison between referral sources, patients and counselors.
• Ensure all discharge planning documentation is clear, concise and timely.

• Develop continuing after care plan, based on patient\'s needs, to promote long term health and relapse prevention.
• Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.
• Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
• Act as a liaison between referral sources, patients and counselors.
• Ensure all patient discharge planning documentation is clear, concise and timely.

• Perform other functions and tasks as assigned.

Education/Experience/Skill Requirements

• Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.
• One or more years’ experience in a clinical setting preferred.
• One or more years’ experience in discharge planning preferred.

Licenses/Designations/Certifications

• If applicable, current licensure as a LPN, RN within the state where the facility provides services.
• CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
• First aid may be required based on state or facility requirements.
