Job summary

Garden City Hospital in Michigan seeks a Full Time Case Management Coordinator to join the team responsible for discharge planning and utilization management. You will coordinate activities across the Case Management Department, assist with referrals and Medicaid submissions, and help develop safe, patient-focused discharge plans.

Ideal candidates have strong computer skills and familiarity with case management terminology.

Qualifications

• High School Diploma required.
• Associates degree or LVN license preferred.

Responsibilities

• Coordinate the activities of the Case Management Department under the supervision of the Case Manager/Social Worker.
• Assist with development and implementation of discharge plans and with referrals and authorizations.
• Maintain Medicaid state submission forms for on-site review and filing.
• Assist with discharge planning by interviewing patients and families to develop safe, patient-focused discharge plans.

Responsibilities

Responsible for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker assist with development and implementations of discharge plans. Assists with case management referrals and authorizations. Maintains and coordinates Medicaid state submission forms for presentation to on-site reviewer and files them accordingly or as per facility's procedure. Assists with Discharge Planning as assigned by interviewing patients/families and gathering data to develop a safe patient-focused Discharge Plan.

Required Qualifications

• Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments.
• Experience in basic to intermediate computer skills/knowledge.
• High School Diploma required.

Preferred Qualifications

• Current BCLS certificate required upon hire and maintain current preferred.
• Associates degree or LVN license preferred.
• Experience in the hospital setting preferred.
