Responsible for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker. Assist with the development and implementation of discharge plans, case management referrals and authorizations, and maintaining and coordinating Medicaid state submission forms. Assist with discharge planning by interviewing patients/families and gathering data to develop a safe patient-focused discharge plan.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $15.49/hr to $20.00/hr. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, based on factors such as skill set, years of experience, education, credentials, and licensure.
