Job summary

Harboroaks in Galax, Virginia, is seeking a discharge planner to develop and manage continuing aftercare plans that promote long-term health and relapse prevention after discharge from the facility. You will coordinate discharge appointments, including therapy sessions with psychologists or psychiatrists, and arrange community resources to support the continuum of care.

Responsibilities

• Develop continuing after care plan, based on patient's needs, to promote long term health and relapse prevention.
• Set discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.
• Arrange for community resources to facilitate discharge and provide all needed documentation to other providers to facilitate the continuum of care.
• Act as a liaison between referral sources, patients and counselors.
• Ensure all patient discharge planning documentation is clear, concise and timely.

Overview

PURPOSE STATEMENT:

Provide assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility.

Hourly rate is negotiable depending on years of experience, education, and other factors. The general hourly pay range for this role is $15-$19.14.

This role does offer a shift differential of $3/hour for hours worked at night.

Responsibilities

• Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.
• Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
• Act as a liaison between referral sources, patients and counselors.
• Ensure all patient discharge planning documentation is clear, concise and timely.

OTHER FUNCTIONS:

• Perform other functions and tasks as assigned.

Qualifications

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

• Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.
• One or more years’ experience in a clinical setting preferred.

• One or more years’ experience in discharge planning preferred.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

• If applicable, current licensure as a LPN, RN within the state where the facility provides services.
• CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.
• First aid may be required based on state or facility requirements.
