Job summary

AHMC Anaheim Regional Medical Center is seeking a Discharge Planner to provide discharge planning services to patients and families across hospital areas, coordinating with the interdisciplinary team and community resources. The role emphasizes patient education, resource management, and adherence to hospital customer service expectations.

Qualifications

  • Bachelor’s degree from an accredited school of Social Work or related field (preferred); RN or LVN (preferred).
  • Two years of acute or related medical care experience.

  • Develops and implements appropriate discharge planning plans of care.
  • Advocates for patient rights.
  • Knowledgeable regarding community resources and mobilizes them on patient’s behalf.
  • Maintains appropriate documentation and statistical records.

Education

Job Overview

The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.
