Job summary

University Health System- San Antonio is seeking a coordinator to assist Social Workers and Case Managers in coordinating patient care across the care continuum. You will collaborate with case managers, physicians, staff, community resources and payers to ensure timely care and efficient use of hospital resources.

Qualifications

High school diploma required. Associates degree preferred. Six months experience in a health care environment required. Preferred experience in HMO, hospital, home health or any clinical setting involved in discharge planning
