Qualifications

• 1 year of experience serving individuals with Serious Mental Illness preferred.
• QMHP-A certification preferred.
• Background checks and driving record verification required.

Responsibilities

• Coordinate discharge planning for individuals hospitalized in state facilities.
• Attend treatment planning meetings and make referrals to community-based placements.
• Document and address individual preferences to promote recovery and community integration.
• Travel locally to various sites to plan discharge and coordinate services.
