EASTERN NEW MEXICO MEDICAL CENTER in Roswell, NM is seeking a Care Management Specialist to provide administrative support for discharge planning, care coordination, and documentation management. You will collaborate with Care Managers, Social Workers, and healthcare providers to ensure timely access to resources and smooth transitions of care.

Strong organizational and communication skills are essential. Experience in care coordination or healthcare administration is preferred, with familiarity

Qualifications

• 1–2 years of care coordination or healthcare admin experience.
• Experience in a hospital or post-acute setting preferred.
• Knowledge of EMR and HIPAA regulations.
• Strong communication and organizational skills.

Responsibilities

• Coordinate discharge planning, referrals, and follow-up services.
• Verify coverage with insurance payers and obtain approvals.
• Document care management activities in the EMR.
• Support patient and family communication and provide discharge information.
• Coordinate with home health, DME, SNFs, and post-acute providers.
• Review patient charts and ensure information is available for staff.
• Track authorizations and follow up on post-discharge needs.
• Assist with patient screenings and ensure regulatory compliance.
• Incorporate age-specific discharge planning considerations.
• Maintain HIPAA compliance and complete documentation.
• Perform other duties and maintain regular attendance.
