Job summary

UHC United Hospital Center is seeking a skilled social worker to coordinate patient transitions across the care continuum. The role involves psychosocial assessments, crisis intervention, and resource management to support successful discharges.

Collaborate with a multidisciplinary team, navigate social programs, and participate in care planning for patients with complex social needs. On-call and weekend rotations are required.

Qualifications

• Masters degree in Social Work required.
• Current social worker licensure as required by the state where work is performed.
• WV: LGSW, LCSW or LICSW licensure through the WV Board of Social Work.
• MD: LMSW or LCSW-C licensure through the Maryland Board of Social Work.
• One to three years of experience preferred.

Responsibilities

• Manages all aspects of transition/discharge planning for assigned patients in a timely manner using escalation processes as needed.
• Collaborates with the multidisciplinary team to facilitate the transition/discharge process for designated caseload.
• Monitors patient progress and ensures the plan of care is patient-focused, high quality, efficient, and cost effective.
• Maintains knowledge of federal, state, and local assistance programs and community resources.
• Provides education to staff, physicians, patients, and families to ensure effective transition planning.
• Meets with patient/family to assess needs and develop an individualized discharge plan with the physician team.
• Provides social work assessment and interventions for complex crises including mental health, substance abuse, and grief/loss.
