Job summary

UPMC Community Medicine Incorporated is seeking a full-time Social Worker for the Shadyside Family Health Center, working Monday through Friday during daylight hours. You will collaborate with a multidisciplinary team of behavioral health and medical providers to support patients and families through psychosocial care and discharge planning.

The role emphasizes brief problem-focused assessments, end-of-life and bereavement support, and timely coordination of services with community resources.

Qualifications

• Provide brief problem focused psychosocial assessments.
• Provide psychosocial support including end of life and bereavement services.
• Assist with discharge planning based on anticipated needs.

Responsibilities

• Demonstrate knowledge of reimbursement systems and educate patients improperly? Wait.
• Demonstrate the ability to educate patients and families regarding payer requirements/coverage for post-acute services and advocate for authorization for post-acute admission.
• Identify, assess, support, and intervene with patients based on medical and psychosocial needs as part of an overall plan of care or during crises.
• Educate and advocate for patients and families to assist in informed medical decisions, discharge planning, and end-of-life care.
• Maintain knowledge of federal, state, local assistance programs and community resources affecting patient needs; connect patients/families with resources/referrals.
• Maintain confidentiality and professional ethics as per NASW Code of Ethics.
• Complete brief psychosocial assessments covering social, economic, cultural, age-related, and other factors.
