Job summary

The Township of Union Public Schools seeks a Transition Coordinator for the 2026-2027 school year. Requires NJDOE certification (TOSD or TOH) and experience with students with disabilities in a school setting. The coordinator will lead transition planning, work with IEP teams, and connect students with postsecondary options.

The role emphasizes collaboration with families, educators, and community partners, while maintaining schedules and documentation to ensure compliance and student progress.

Qualifications

• Must hold NJDOE Certification (TOSD or TOH).
• Experience with students with disabilities in educational settings.
• Knowledge of N.J.A.C. 6A:14, IDEA, and NJDOE transition requirements.
• Knowledge of transition planning and services for students with disabilities.
• Ability to collaborate with families, educators and community partners.
• Strong organizational and communication skills.
• Ability to maintain records and manage deadlines.

Responsibilities

• Coordinate and support the district's transition planning and services.
• Collaborate with case managers and IEP teams to support compliance with transition requirements.
• Develop individualized transition plans based on student strengths and needs.
• Attend conferences with parents/guardians and staff to review goals and activities.
• Support development of measurable postsecondary goals in education/work.
• Coordinate transition-related assessments, activities and services.
• Establish partnerships with local businesses and community organizations.
