Job summary

Montefiore Medical Center is seeking a supervisory professional to oversee SSA Disability Services (FDS) case management, ensuring quality documentation for SSI/SSD applications. You will lead FDS team efforts, coordinate with medical and psychiatric providers, and ensure compliance with NYC HRA contracts.

The role requires an associate degree (bachelor’s preferred), 3 years of supervisory experience, and strong written/verbal communication, with proficiency in SEAMS, EPIC, and Microsoft Office.

Qualifications

• Associate degree required.

• Medical/behavioral health diagnoses may impact functional capacity.

Responsibilities

• Oversee SSA Disability applications and case management for the FDS Team.
• Develop relationships with medical and psychiatric providers in New York to obtain documentation.
• Ensure compliance with NYC HRA contractual requirements.
• Assist in staff training and development materials.
