The Disability Services Case Manager role in Davenport, IA supports individuals with ID, DD, BI, HD, PD, and other waivers to access essential services. Travel across multiple Iowa counties is required to ensure resources are accessible and appropriate.
Base location is Davenport with significant travel to Johnson, Des Moines, Muscatine, Washington, and more; candidates must reside in Iowa. Strong communication and regulatory interpretation skills are essential for success in this role.

The Iowa Department of Health and Human Services, Division of Aging and Disability Services, Bureau of Community Integration, Targeted Case Management is seeking a Disability Services Case Manager to join our team!
This position will be based in Davenport, Iowa but there will be significant travel to Johnson, Des Moines, Muscatine, Washington and several other Iowa Counties, helping individuals with disabilities access to resources they need to thrive. You must live in the State of Iowa to be eligible for this position.
Hours for this position vary based on the needs of the individuals we support, Monday – Friday. Travel is based on the needs of the individuals supported and in-home visits are required across multiple Iowa counties. Mileage is paid at $0.50/mile travelled according to the Department of Administrative Services (DAS) State Accounting Enterprise Procedures. There is very limited overnight travel.
A valid driver’s license and a reliable vehicle are required for this position.
The Disability Services Case Manager works with individuals with Intellectual Disabilities (ID), Developmental Disabilities (DD), and individuals utilizing the Brain Injury (BI), Health and Disability (HD), Physical Disability (PD), Elderly and AIDS/HIV waivers. They help identify service needs, obtaining them, and evaluate progress in the provision of those services.

Applicant must have Iowa license to practice as a registered nurse AND the equivalent of 3 years nursing or human services experience.Graduation from an accredited four-year college or university; the equivalent of four years of full-time technical work experience involving direct contact with people in overcoming their social, economic, psychological, or health problems; an equivalent combination of education and experience substituting the equivalent of one year of full-time qualifying work experience for one year (thirty semester or equivalent hours) of the required education to a maximum substitution of four years.
