Job summary

Apploi's Staten Island facility seeks a Director of Social Work (DSW) to lead the Social Services Department, guiding resident assessments, care planning, and transition support in a warm, patient-centered environment.

The role requires an LSW or MSW with at least 2 years of supervisory experience in long-term care. You will mentor staff, manage discharge planning, and engage in community outreach to strengthen local partnerships.

Qualifications

• Requires LSW or MSW licensure.
• 2+ years of supervisory or management experience in LTC.
• Strong knowledge of state and federal healthcare guidelines.
• Excellent interpersonal and leadership abilities.

Responsibilities

• Direct, establish, and plan department goals, policies, and procedures.
• Oversee resident assessments, care planning, and transition support.
• Lead, mentor, and supervise Social Services staff.
• Manage discharge planning from admission onward.
• Occasional community outreach to foster partnerships.
• Apply clinical judgment and best practices to quality of care.
