Affinity Skilled Living and Rehabilitation Center in Sayville, NY seeks an experienced Director of Social Work/Case Management for a premier skilled nursing facility. The role requires Medicare/Medicaid knowledge, MLTC, HMO Case Management, and discharge planning, with NYS license and solid nursing home experience.

You will lead the Social Work team, coordinate patient care, ensure regulatory compliance, and support successful transitions from hospital to subacute care.

Qualifications
