Job summary

Executive Search Network/Platinum Staffing is seeking a highly skilled Director of Social Work to join our team at a skilled nursing facility in Suffolk County, New York. You will lead and manage a team of social workers and provide exceptional social services to residents.

This role requires a Bachelor's degree in Social Work and at least two years of experience as a Social Worker in a Nursing Home, with strong written and verbal communication skills.

Responsibilities

• Assist new residents and families during admission and transitions to institutional living.

• Interpret residents' social, psychological and emotional needs to help plan goals with the resident and facility.
• Refer residents to appropriate social services and arrange transportation when needed.

Job Summary

Executive Search Network/Platinum Staffing is seeking a highly skilled and experienced Director of Social Work to join our team at a skilled nursing, long-term care facility located in Suffolk County, New York. As a key member of our team, you will be responsible for providing exceptional social services to our residents, as well as leading and managing a team of social workers.

Job Responsibilities

• Assist new residents and their families in the initial adjustment phase, as well as in the ongoing transition to institutional living
• Provide information to residents and/or family members on residents' rights and responsibilities
• Interview residents/families to obtain social history, and participate in discharge planning, development and implementation of social care plans and resident assessments
• Interpret social, psychological and emotional needs of the resident care team members, and involve the resident/facility in planning objectives and goals for the resident
• Refer residents/families to appropriate social services agencies as needed and arrange transportation to other facilities when necessary

Qualifications/Requirements

• Must have experience as a Social Worker in a Nursing Home
• Strong written and verbal skills
• Excellent communication skills
