Job summary

The McGregor Foundation is seeking a Director of Social Services for our PACE program in Ohio. You will lead the Social Services team, conduct psychosocial assessments, and ensure quality care for older adults and their families.

This is a hands-on leadership role reporting to the PACE CEO and requires licensure as LSW/LISW and an MSW. Responsibilities include developing staff, building community partnerships, coordinating services, and guiding discharge planning while maintaining regulatory

Qualifications

• Requires a bachelor’s degree in social work or related field.
• 1–2 years of healthcare experience, preferably in long-term care.
• LSW or LISW licensure is required.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Conduct psychosocial assessments and care conferences with residents and families.
• Facilitates treatment planning, decision-making, and communication among residents, families, and healthcare teams.
• Educates residents and families about illness, treatment options, and community resources.
• Coordinates ancillary services like psychological and psychiatric support.
• Advocates for residents' rights and ensures effective discharge planning.
• Participates in facility quality improvement and staff training.
• Maintains documentation compliance and manages resident concerns.

About the Role

Reporting to the PACE CEO, we are seeking a Director of Social Services who is a MSW and licensed as either LSW or LISW in the state of Ohio to lead and develop our Social Services team. This is an opportunity to lead clinical practice, develop staff, build community partnerships, and influence organizational strategy while making a meaningful difference in the lives of older adults and their families.
