Job summary

Covenant Retirement Communities & Covenant Care is seeking a licensed Social Worker to lead the social services team at our Evanston campus. The Social Services Director will assess residents, coordinate care planning, and advocate for resident rights, working closely with families and the care team.

The role requires experience in long-term care, supervisory experience, and strong communication skills to support residents, families, and staff across multiple levels of care.

Qualifications

  • Licensed Social Worker in the state
  • Bachelor's degree in Social Work (Master's preferred)
  • Four years of experience in medically related long-term care
  • Two years in a supervisory capacity in social work
  • Knowledge of community resources
  • Excellent verbal and written English communication
  • Flexibility, adaptability, and attention to detail
  • Ability to handle highly confidential information and maintain trust

Responsibilities

  • Assist as facility representative with prospective residents, families and discharge planners for skilled nursing care needs
  • Provide social assessments of residents on admission and identify psychosocial needs
  • Assist with orientation of residents to the facility and services
  • Record admission data including social, medical and financial information
  • Maintain documentation in residents' health records for the social services component
  • Participate in resident care planning meetings for social services delivery
  • Assist residents/families in selecting new physicians when needed
  • Communicate information to departments regarding admission or discharge
  • Advocate for residents and families and uphold resident rights
  • Maintain regular family contact and liaison with the care team
  • Establish regular family support meetings for medically related social/emotional needs

This position is responsible for directing the social service functions which include the planning, development and implementation of appropriate programs designed to address the emotional and psychological needs of campus residents in multiple levels of care and their families. This position is responsible for actively participating in the resident assessment process and in assisting the facilitating of inter-facility transfers. This position serves as a resident advocate within the campus.In this role, the Social Services Director will:Assist as the facility representative with prospective residents, families and discharge planners for persons in need of skilled nursing care.Provide social assessment of residents upon admission and identify psychosocial needs in compliance with State and Federal Regulations.Assist with orientation of residents to the skilled nursing facility and services.Collect and accurately record admission data including social, medical and financial information and address questions and concerns and family adjustment.Maintain current documentation in resident’s health record as it relates to the social services care component of the comprehensive care plan for each resident as identified by the interdisciplinary team.Assure that appropriate care plan documentation as required by regulations and need is complete.Participate in resident care planning meetings for ongoing departmental communication and problem-solving of delivery of Social Services to residents and families.Assist residents and/or families in selecting new physicians as necessary.Assist in the communication of appropriate information to departments regarding admission or discharge.Function as an advocate for residents and/or families in ensuring resident rights are carried out.Provide regular contacts with families and act as liaison for families with the resident care team.Establishes regular family support meetings pertaining to medically related social and emotional needs or care.Explain government and facility policies to new residents/families.The ideal candidate will have:License as a Social Worker in the State.Bachelor’s degree in Social Work; Master’s degree in Social Work preferred.Four years experience in working directly with individuals in a medically related field with long-term care experience.Two years in a supervisory capacity in the social work field.Knowledge of community resources.Excellent communication skills, verbal and written, in English, with tact, professional manner and confidence in the personal contact with residents, family members and general public.Flexibility and adaptability.Good organizational skills with particular attention to detail and accuracy.Ability to follow through on assigned tasks, and demonstrate good judgment and initiative.Ability to work with highly confidential information and establish positive, trusting relationships.#LeadershipCompensation Pay Range:$67,295.00 - $93,610.25 per yearReasonable Pay EstimateA reasonable estimate of the pay range for this position is $67,295.00 - $93,610.25 per year.
