Evergreen Healthcare Group is seeking a Director of Social Services to lead the Social Services Department in a skilled nursing setting. The role focuses on advocating for residents and guiding families through transitions as part of a cohesive interdisciplinary team.

The ideal candidate holds a BSW, with a Master’s degree preferred, and a current LSW certification. Expect approximately 35–38 hours per week, with comprehensive benefits and the opportunity to lead quality of care initiatives.

Responsibilities

• Lead and oversee the Social Services Department.
• Advocate for residents and ensure their emotional and psychosocial needs are met.
• Build strong relationships with residents and their families.
• Help residents and families navigate the transition into long-term care.
• Participate in care planning and discharge planning.
• Coordinate transitions of care and connect residents with resources.
• Partner with nursing, therapy, activities, dietary, and other departments as part of the interdisciplinary team.
• Maintain assessments, documentation, progress notes, and care plans.
• Ensure compliance with state, federal, and facility requirements.
• Provide leadership and guidance to social services staff.
