Job Description
Position Overview: This position is responsible for managing the development, implementation, and evaluation of standardized systems, processes, and techniques, as well as the creation of policies and procedures. Key responsibilities include identifying and assessing potential risks to systems (such as patient safety and processes), minimizing or preventing losses and risks, achieving quality assurance, performance improvement, and safety improvement goals, ensuring compliance with regulatory and accreditation standards, and enhancing the quality of care provided to residents. Additionally, this role supports the Associate Executive Director in their efforts to deliver high-quality, equitable care to patients while improving workflow efficiency for all staff members.
Responsibilities
- Collaborates with clinical and non-clinical departments to implement risk reduction strategies to eliminate threat to safety.
- Screens and investigates accidents and incidents involving safety matters and promptly reports to the appropriate local, state and federal public health and regulatory agencies. Analyzes and documents accidents and incidents, utilizing appropriate tools and other applicable methods of analysis, to identify contributing factors and other safety concerns.
- Manages risk identification “and assessment through data collection and analysis, investigating near misses, adverse occurrences, and sentinel events within the facility.
- Utilizes standard classifications to categorize incidents and participates in patient safety committees.
- Assists in reporting to corporate committees and regulatory agencies like the New York State Department of Health. Conducts internal audits and takes corrective actions for identified deficiencies.
- Designs, develops, and implements effective strategies and programs throughout the health care setting
focused on improving quality, patient experience, and population health outcomes and reducing the cost of patient care.
- Serves as both principal advisor and key resource to senior leadership, clinical staff, and other appropriate parties on matters pertaining to quality management techniques and systems, and presents findings of assessments, analyses, and safety incidents to various agencies, executives and/or senior managers. Advises on upcoming regulatory changes and required actions needed to meet or exceed new industry standards and to achieve improved outcomes.
- Promotes a safety culture and ensures compliance with laws and organizational policies, encouraging a non-punitive error reporting system.
- Supports the Associate Executive Director in investigations and regulatory reporting. Assists in developing and monitoring the annual risk assessment and safety plan.
- Leads Quality Reviews, RCAs, FMEAs coordinating with the teams to enhance care quality and safety.
- Models a patient-centered approach and assists the Associate Executive Director in reviewing SNF metrics and collecting relevant data.
- Differentiates between normal and abnormal health findings, conducts literature reviews, and provides education to nursing staff.
- Develops scorecards and dashboards for quality tracking, attends QAPI committee meetings, and utilizes system-wide tools for reporting.
- Schedules performance improvement meetings and assists with Annual Recertification (LTACH/SNF) preparations through quality rounds, audits, and compliance monitoring.
- Serves as a principal advisor and key resource to senior leadership and other parties, and as liaison
between appropriate agencies, on all matters pertaining to patient and public safety, risk assessment, and other factors relating to security excellence.
- Assists in the development of training programs and provides orientation, training, and guidance to staff. Routinely assesses staff learning needs, identifying gaps and evaluating the quality and effectiveness of the programs. Implements necessary changes in trainings and development to maximize productivity, effectiveness and efficiency of staff.
- Performs other related duties, as directed.
Minimum Qualifications
- Master’s degree from an accredited college or university in Risk Management, Health Administration, Business Administration or in a related health care specialization, and three (3) years of experience in risk management, control, and prevention, leading and implementing quality and risk management programs, managing and analyzing risk data, and performing other functions related to risk management in a hospital or a health care setting, one (1) year of which must have been in a responsible administrative, managerial or supervisory capacity; or
- Bachelor’s degree from an accredited college or university in fields, as described in “1” above, and four (4) years of risk management experience, as described in “1” above, two (2) years of which must have been in a responsible administrative, managerial or supervisory capacity; or 3. Bachelor’s degree in the disciplines as described in “1” above; and two (2) years of clinical, patient care or equivalent experience in a hospital or health care setting, plus two (2) years of related experience, as listed in “1” above, one (1) year of which must have been in a responsible administrative, managerial or supervisory capacity; or 4. A satisfactory equivalent of education, training, and/or experience. Certified Professional in Healthcare Risk
Management (CPHRM) or current license and registration to practice as a Professional Nurse, Nurse Practitioner or other similar clinical designation, may be substituted for one (1) year of experience, however, all incumbents must have at least a bachelor’s degree and one (1) year of responsible administrative, managerial or supervisory experience, as described in “1” above.
Department Preferences
- Licensed and currently registered to practice as a Professional Nurse in New York State, plus (2) or (3) below.
- A Master's Degree in Nursing Administration, Public Administration, Public Health or Health Care Specialization or an equivalent field and a minimum of six years of satisfactory nursing experience, of which two years shall have been in a supervisory, teaching or administrative capacity; or,
- A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services; or,
- A satisfactory equivalent combination of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State
Clinical experience in coordinating and monitoring initiatives, activities, and programs related to patient safety and/or quality improvement or performing other functions directly related to patient safety in a hospital or health care setting
Nationally recognized quality/safety certification such as CPHQ and/or CPPS.
Excellent interpersonal and communication skills
