Job summary

Prava Therapy is seeking a Director of Rehabilitation to lead a comprehensive program in Northern California (Folsom & Elk Grove). This role builds a community-integrated rehabilitation program, leads clinical and operational teams, and partners with executive leadership to align rehab with residents’ wellness and independence.

Ideal candidates are California-licensed clinicians (PT/OT/SLP) with leadership experience, a data-driven mindset, and a passion for mentoring clinicians to deliver

Qualifications

• Licensed rehabilitation clinician in California.
• Experienced PT, OT or SLP with leadership ability.
• Comfortable leading clinicians while maintaining your own clinical practice.
• Strong clinical thinker who cares about quality.
• Relationship builder with senior living leadership.
• Mentoring and developing other clinicians.
• Organized, accountable and comfortable using data to manage performance.
• Leadership ability, clinical judgment and the desire to build a great program.

Responsibilities

• Provide direct physical, occupational and/or speech therapy as appropriate.
• Establish a culture of evidence-based practice and resident-centered care.
• Review documentation and clinical performance.
• Support clinicians with complex cases and clinical decision-making.
• Ensure evaluations, plans of care, progress reports and discharge planning reflect resident goals.
• Promote high-quality outcomes.
• Recruit, onboard and develop rehabilitation clinicians.
• Create accountability around productivity, documentation, scheduling and clinical quality.
• Mentor therapists and assistants.
• Lead regular clinical and operational meetings.
• Develop strong relationships with executive directors and wellness leadership.
• Identify residents who may benefit from therapy and build referrals.
• Partner with wellness and fitness programming within the community.
• Grow the program with analytics and new service lines.
