Job summary

Continuum Therapy Partners is seeking a Director of Rehab to lead our rehabilitation services across facilities. The role focuses on directing and optimizing the rehab program, ensuring compliant, high-quality care, and building strong relationships with facility leadership.

Ideal candidates will hold current licensure and have experience in managing therapy services across multi-site environments, with a focus on PT/OT/SLP disciplines. This is a full-time, on-site leadership position.

Qualifications

• Graduate of an accredited therapy program (PT/PTA/OT/COTA/SLP).
• Current state licensure in the applicable therapy field.
• Plan, schedule, direct, and oversee all rehabilitation services.
• Develop and maintain relations between Continuum staff and facility leadership.

Responsibilities

• Provide professional support services to residents in rehabilitation centers.
• Initiate referrals per Therapy Department procedures.
• Oversee rehabilitation operations to ensure program effectiveness.
• Drive program development objectives and monitor outcomes.
