Job summary

CommuniCare Rehab, part of the CommuniCare Family of Companies, seeks a Director of Rehabilitation for Ivy Woods Healthcare Center in Cincinnati, OH. This full-time leadership role oversees therapy services, ensures quality patient care, and drives clinical programming with an in-house team.

The Director will lead evaluations, treatment planning, and staff education while coordinating with facility and corporate teams to maintain compliant, high-quality rehab services for residents.

Qualifications

• Must be a state licensed clinician (PT, OT, SLP, PTA, or COTA) from an Accredited Program.
• Excellent oral and written communication, and effective leadership skills.

Responsibilities

• Performs evaluations, screens, and treatments of facility patients/residents as needed.
• If evaluating therapist, perform patient evaluation under physician order and develop a treatment plan.
• Implements treatment plan and educates staff involved in the plan.
• Attends or leads interdisciplinary meetings and supports clinical programming.
• Relays clinical findings and makes recommendations to rehab team.
• Collaborates with facility and Denials Management on ADR/denied therapy claims.
• Conducts staff and family training and education via in-services and consultations.
• Maintains accurate billing information and complies with documentation requirements.

Responsibilities

• Performs Evaluations, Screens, and treatments of facility patients/residents who are in need for Therapy services.
• If an evaluating therapist, perform patient/resident evaluation under physician order and develops a treatment plan in accordance with patient’s needs.
• Implements treatment plan through direct treatment, education of treatment staff which includes therapists and other members of team involved in the treatment plan.
• Attends/leads interdisciplinary meetings as appropriate, as well as function as a resource for clinical programming and caseload.
• Will also relay clinical findings and make recommendations to other members of the rehab team.
• Collaborates with facility and corporate Denials Management Specialist regarding any ADR/denied therapy claims.
• Conducts staff/family training and education when appropriate by formal and informal in-services as well as consultations.
• Maintains accurate billing information in accordance with facility procedures daily, including training/re‑training of team members, and complies with documentation requirements for rehab department.
