Job summary

Total Care Therapy is seeking a Director of Rehabilitation - Physical Therapist to lead rehab services in Powell, OH. You will oversee clinical operations, supervise PTs and PTAs, and ensure evidence-based care that improves mobility and independence for older adults in assisted living and skilled nursing settings.

Ideal candidates will hold an active PT license, CPR/BLS, and demonstrate strong leadership, communication, and organizational skills.

Qualifications

• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred.
• Previous leadership, supervisory, or Director of Rehabilitation experience preferred.
• Strong communication, organizational, and leadership skills.

Responsibilities

• Provide skilled physical therapy services according to established plans of care.
• Deliver therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions.
• Monitor patient progress and modify treatment plans as needed to achieve functional goals.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering compassionate, high-quality therapy services.
• Act as the main liaison between leadership and the facility.
• Oversee staff performance and support professional development.
• Manage administrative and operational tasks across therapy services.
• Coordinate meetings, training, and events for staff and residents.
• Maintain communication with residents, families, and leadership.
• Drive referrals for all disciplines
