Job summary

Onyx Care of Ruston is seeking a proactive Rehabilitation Leader to oversee therapy operations across multiple facilities. The role includes supervising DORs and staff, ensuring licensure-based treatment, and aligning services with Innovate Rehab & Wellness standards while meeting federal and state regulations.

The position requires leadership in long-term care rehab, strong clinical knowledge, and collaboration with administrators to improve productivity and outcomes across sites.

Qualifications

• Licensed PT/OT/SLP/PTA or COTA/OTA required.
• 4–6 years in long-term care rehabilitation with at least 2 years in a leadership role.
• Multi-site management experience preferred.
• Strong knowledge of Medicare/Medicaid regulations and SNF compliance.

• Supervise, mentor, and support facility-level DORs and therapy staff.
• Provide treatment as appropriate to professional designation/licensure.
• Ensure therapy services meet Innovate Rehab & Wellness standards, as well as federal, state, and payer regulations.
• Monitor productivity, quality outcomes, and financial performance across facilities.
• Partner with facility administrators and nursing leadership to address operational challenges.
• Conduct site visits, audits, and compliance checks; implement corrective action when needed.
• Support recruitment, onboarding, and professional development of therapy leaders.
• Report area performance metrics to Area Director or VP of Operations.
