Job summary

Gateway at River City is seeking a passionate Director of Nursing & Wellness to lead the clinical team in delivering high-quality care to residents. The role oversees care plans, medication management, and regulatory compliance while fostering a collaborative environment with physicians, families, and staff.

The position requires RN licensure in Illinois (in good standing) or eligibility, 2+ years of nursing leadership, budgeting skills, and proficiency with care coordination tools.

Qualifications

• RN licensure in Illinois (in good standing) or eligible
• Minimum of 2+ years as a nurse leader in senior living
• Strong budgeting and financial reporting understanding
• Proficiency with Microsoft Office and care coordination software
• Valid CPR, TB and First Aid certifications
• Valid driver’s license with reliable vehicle
• Ability to manage multiple tasks in a fast-paced environment

Responsibilities

• Ensure resident care and wellness needs are met
• Maintain accurate resident medical records and documentation
• Conduct initial assessments of prospective residents and home/hospital visits
• Develop and update resident care plans
• Manage IHCC hours and service level documentation
• Oversee budget related to department staffing and supplies
• Recruit, train, and supervise the clinical team (LPNs and CNAs)
• Coordinate medication management and communicate with physicians and families
• Lead quality assurance and regulatory compliance initiatives
• Promote health and disease prevention, and infection control programs

Gateway at River City is looking to add a passionate, dedicated Director of Nursing & Wellness to our team! The Director of Nursing & Wellness leads the clinical team in delivering quality care and service to residents.

What You'll Be Doing To Inspire Others

• Ensures resident care and wellness needs are met
• Ensures resident medical records are accurate and complete, including monitoring documentation and task completion
• Conducts initial assessment on prospective residents, including off-site visits (I,e, home visits, hospital visits)
• Conducts assessments and develop resident care plans with routine updates
• Captures Integrated Health Care Coordination (IHCC) hours through documentation
• Submits documentation to AAA regarding level of services changes and reconciles level of service discrepancies
• Establishes and manages relationships with third-party relationships and resources
• Maintains spending to established department budget including labor and supplies
• Recruits, trains, schedules, and manages clinical team including LPNs and CNAs
• Coordinates and supervises the community’s medication management program
• Communicates with resident’s physician(s), other pertinent health care providers, and family as to the ongoing health status of the residents.
• Participates in quality assurance and improvement initiatives
• Reports and resolves of regulatory issues and/or concerns in a timely manner
• Provides health promotion services, disease prevention counseling, and teaching self-care to residents on an as needed basis
• Coordinates the facility’s overall infection control program

Your Qualifications For Success

• High School Diploma AND
• Two (2) or more years of experience as a nurse leader in a senior living environment
• Illinois nurse’s license, in good standing, RN preferred
• Valid CPR, TB, and First Aid Certification
• Valid driver’s license with acceptable driving record and proof of automobile insurance
• Working knowledge of Illinois Supportive Living Regulations
• Understanding of basic budgeting and financial reporting
• Strong organizational skillsAbility to build strong relationships with internal and external contacts
• Ability to manage multiple tasks and projects in a fast‑paced, dynamic environment
• Strong attention to detail
• Proficiency with Microsoft Office products and web proficiency
• Proficiency with Point Click Care, preferred
